The role of the mesoscooter in the anti-age program

The use of a mesoscooter is increasingly common in the practice of estheticians and cosmetologists. Let's consider the combined use of a mesoscooter with other methods of aesthetic correction in the anti-age program.

Human skin is influenced by many factors, which can be divided into internal and external. Therefore, in cosmetology it is customary to distinguish two types of aging: chronoaging and photoaging.


During chronoaging (biological, age-related), the following processes predominate in the skin:

  • slowdown of regeneration;
  • reduction in the thickness of the epidermis;
  • flattening of the border between the dermis and epidermis;
  • reduction in the number of Langerhans cells and melanocytes;
  • decrease in the amount of collagen (annually by 1%), elastin and ground substance (due to a decrease in the number of fibroblasts);
  • reduction in the number of blood vessels.

Photoaging (associated with exposure to environmental factors, especially ultraviolet radiation) manifests itself on the skin in the form of:

  • hyperkeratosis;
  • age spots (solar lentigo);
  • elastosis (thickening of the dermis due to increased synthesis of altered elastic fibers);
  • a decrease in the amount of collagen in the dermis (associated with the destruction of its MMPs, which are activated under the influence of UV rays);
  • obliteration of some vessels, while the rest significantly expand, forming telangiectasia;
  • damage to Langerhans cells, decreased immunological defense.

Depending on age, type of skin aging, and constitutional features, the following visual changes appear:

  • drooping tissue (ptosis);
  • sagging, wrinkling;
  • deepening of the nasolabial fold, wrinkles in the corners of the mouth;
  • the appearance and deepening of facial wrinkles (forehead, bridge of the nose, “crow’s feet”);
  • violation of the oval of the face, the appearance of a double chin;
  • the appearance of pigment spots;
  • sharpening of facial features;
  • dark circles and puffiness under the eyes;
  • rosacea

As a result of these processes, an integrated approach is required to rejuvenate, slow down skin aging by stimulating collagen production, improving microcirculation, providing antioxidant and restorative effects, and effectively affecting skin trophism.

General principles of correction


Based on the signs of aging, anti-age therapy usually solves the following problems:

  • increased tone and elasticity;
  • improvement of microcirculation and lymphatic drainage;
  • elimination of local lipodystrophy (double chin);
  • creating a frame and tension lines;
  • eliminating facial stress on individual zones;
  • replenishment of soft tissue volume;
  • relief leveling;
  • restoration of the epidermal barrier;
  • elimination of hyperpigmentation.

To achieve these goals, aesthetic and medical cosmetology procedures are used. Moreover, the success of cosmetic procedures depends on the consistency and consistency of their implementation.


As practice shows, a logical plan for correction of the face, neck, and décolleté can be as follows: first, needling with a mesoscooter (1–2 procedures), a course of mesotherapy (6–10 sessions according to the scheme) or biorevitalization (3–4 procedures), then correction of facial wrinkles botulinum toxin preparations and after 2-3 weeks the introduction of fillers. Let's consider the step-by-step implementation of such a correction and dwell on the features of each stage.

Microneedling


Microneedling, also called dermal needling (or rolling), is a technique that involves stimulating the upper layers of the dermis with microneedles, with or without mesotherapy solutions. The procedure is performed using a mesoscooter - a sterile medical device consisting of a cylindrical head with 360-540 very thin needles and an ergonomic handle. Thin needles create many microscopic channels in the skin.

During the needling procedure (acupuncture), the following occurs:

  • stimulation of collagen induction;
  • improvement of microcirculation;
  • formation of microholes for the introduction of bioactive drugs;
  • relief leveling;
  • stimulation of reparative processes.

That is, the effect of a mesoscooter has a pathogenetic effect on the aging process or on those structural changes that occur in the skin with age. Also, these channels allow substances contained in medicinal preparations to penetrate more deeply (if mesotherapy cocktails are used during the procedure).


Before the procedure, local anesthesia with Emla cream is applied for 20–30 minutes. After appropriate treatment of the skin with antiseptic solutions, a needling procedure is performed with a mesoscooter in the vertical, horizontal and diagonal direction in the area of the face, neck and décolleté. The length of needles for anti-age correction of the face is 1–1.5 mm, for post-acne scars – 1.5–2 mm. During the “dry” procedure, no cocktails are injected into the skin. After the procedure, hyperemia lasts 24–48 hours. If another procedure is needed, it can be done after 6–8 weeks.


The mesoscooter has been used in cosmetology practice not so long ago, but it cannot be said that this is a new device. In Chinese traditional medicine, reflex mechanical effects of such devices have been used for a long time. It consists of superficial multiple acupuncture of certain areas and biologically active points (reflex reaction) and micromassage of the skin and underlying tissues. The local reaction is expressed in changes in the blood supply to the skin area, temperature, and sensitivity.

Biorevitalization


The next logical step after needling may be biorevitalization. A lot has been said about biorevitalization; it is a proven effective correction method. With intradermal administration of hyaluronic acid (HA), stimulation of the dermal matrix, synthesis of endogenous HA, neocollagenogenesis, neoangiogenesis, replenishment of skin hydration, and a powerful antioxidant effect occur.


For the procedure, unstabilized, high-molecular pure HA or in combination with mannitol, glycerol, and vitamins can be used. Being a polyanion in structure, HA is capable of binding and retaining large amounts of water through hydrogen bonds. Mannitol, which acts as a strong antioxidant, delays the action of free radicals and hyaluronidase on HA by 3 times.


In combination with a mesoscooter, biorevitalization is carried out 2–4 weeks after needling.


To restore the matrix structure, the optimal ratio of molecular weight and concentration is critical. The combination of a concentration of 10 to 15 mg/ml and a molecular weight of over 2.5 kDa forms a molecular network that creates a reserve of HA in the extracellular matrix and stimulates intracellular synthesis.


When performing biorevitalization in the area of the face, neck, and décolleté, along with the papular technique, the superficial linear technique (controlled intradermal destruction) and lattice technique are often used. Areas of depression and wrinkles and the parotid fold are especially actively treated.


As a result of the correction course in these areas, the following are visually determined:

  • pronounced lifting effect;
  • tightening and filling the skin;
  • reduction of fine wrinkling and depth of folds;
  • even out skin color;
  • smoothing the relief.

Mesotherapy and mesoroller


The mesoscooter can also be used as a device for administering mesotherapy drugs. When “rolling” with a mesoscooter, micro-holes are formed in the skin, through which the drug penetrates deep into the skin. This introduction of drugs ensures their uniform penetration to the same depth. Also, the combined stimulating effect of the mesoscooter itself and the substances administered with its help has a positive effect. Depending on the drugs administered, excellent effects are achieved when working with both the face and body.

When using different lengths of mesoscooter needles, in addition to anti-age therapy, it is possible to correct the following problems:

  • hair loss (needles length 0.5–1 mm);
  • scars (1.5–3 mm);
  • stretch marks (1.5–3 mm);
  • cellulite (2–3 mm).

Botulinum toxin injections

Why does the sequence of “mesotherapy – Botox” procedures look exactly like this? The fact is that first it is necessary to improve the quality indicators of the skin: tone, turgor, density, elasticity. That is, start the work of fibroblasts, increase the amount of matrix with the help of hyaluronic acid, and strengthen the vascular wall.

Mesotherapy is an actively stimulating procedure; accordingly, the effect of botulinum toxin injection will appear faster, since a new branch of the nerve innervating the muscle will grow in a shorter period of time.


Anti-age mesotherapy uses active regenerating and stimulating cocktails that improve the quality of the skin. The synthetic and proliferative work of fibroblasts is stimulated, the number of collagen and elastin fibers and matrix increases, microcirculation improves, and oxidative stress is eliminated. Preference is given to cocktails with growth factors (EGF, IGF-1, bFGF, VEGF) and biostimulins (Dermaheal, Mesoline).

Also included in mesotherapy cocktails for anti-age therapy are:

  • vitamins (C, A, B5);
  • trace elements (silicon, zinc, copper, selenium);
  • vasoactive drugs (nicotinamide, ginkgo biloba, rutin meliloto);
  • nucleic acids and amino acids;
  • antioxidants;
  • biostimulins and growth factors (placentex, Asian centella extract, embryoblasts);
  • hyaluronic acid.

The drugs are administered nappage, papularly and linearly. The course is 6–10 procedures once a week or according to the scheme.


The use of a 0.25–0.5 mm mesoscooter with medications is possible once a week. After anesthesia, one passage is performed with a mesoscooter, then irrigation from a syringe with a mesopreparation, and so on.


Autoplase (PRP) with platelet-derived growth factor is injected deep into the tissue and superficially papular. Course – 4–6 procedures once every 14 days.


For hyperpigmentation, whitening cocktails with a mesoscooter are used.


In the lower third, not only the usual tightening, but also lipolytic cocktails are used: silicon 0.5% + hophytol + procaine, L-carnitine + procaine, phosphatidylcholine + deoxycholinic acid and ready-made cocktails for the treatment of local lipodystrophy (Mesoline, Inno-TDS, mesoEXPERT) . In this case, the skin is folded so that the solution gets directly into the hypodermis. Depending on the cocktail, 6–8 procedures are required with a frequency of 7–10 days or 2–4 procedures every 10–14 days.

Botulinum toxin injections


Most often, botulinum toxin is injected into the upper part of the face: forehead, bridge of the nose, periorbital area. In the lower third, a block of the depressor anguli oris muscle is used to correct grief lines. Also in this area, above and below the lower jaw bone, botulinum toxin injections are made using the papular technique in double dilution. As a result, we get a clearer oval - “Nefertiti oval”.

Fillers


In combination with contour plastic surgery, it is possible to use needling with a mesoscooter (1-2 procedures), then a 4-week break, filler injections, and a month later - again with a mesoscooter. With the help of this set of procedures, the following effects are achieved: relief leveling, lifting, uniform filling.


Most often, indications for contour plastic surgery are requests for the correction of nasolabial folds, “purse-string” wrinkles around the mouth and “sorrow” wrinkles in the corners of the mouth. More “advanced” correction is the nasolacrimal groove, brow ridges, cheekbones. Preference is given to natural biodegradable fillers based on hyaluronic acid.


To create a tension effect, it is necessary to inject fillers into the parotid fold. In this zone, by the age of 30–35, a fold forms, and by the age of 40, several folds or a network of wrinkles. The correction is as natural as possible and gives a slight lifting effect. There is no hypercorrection (you have to try), since all the material fits directly into the fold.


In areas of depression on the cheeks, a lattice frame is formed with a needle or cannula (0.5–1.0 preparations on each side). Administration of the drug by cannula is preferable - it is a less traumatic and more “large-scale” method. This type of correction can be supplemented by introducing filler into the zygomatic area - a pinpoint injection of a “bump” or a linear “grid” method.


Aligning the oval with the mandibular bone can be done by placing filler in the pits near the “bulldog cheeks”. It is possible to inject the most flexible fillers (Pluryal) into neck wrinkles, especially under the chin.


In addition to correcting signs of aging on the skin of the face, an anti-age program for the skin of the hands is possible using a mesoscooter and fillers. You can perform needling and volumetric filling in one procedure: after anesthesia, perform contouring, “place” the drug and perform needling with a mesoscooter.


After 2–3 weeks we get a rejuvenating effect:

  • filling;
  • volumetric correction and, as a result, reduction in contouring of veins on the hands;
  • relief leveling;
  • reduction of wrinkles and depth of folds;
  • lifting;
  • color alignment.

Thus, a specialist in the field of cosmetology must be aware of the latest innovations in the industry and use them wisely in his practice, because regular care and adequate correction using modern methods can prolong youth and prevent the appearance of new signs of aging.
