Mesotherapy in aging correction programs

For women of Slavic appearance, mesotherapy is a basic tool in solving aging problems. A correctly selected procedure protocol will ensure the expected result for your patient.

Attractive appearance has always been, is and will be one of the most important criteria that attracts people. Nowadays, in competition for a “place in the sun,” both women and men try to meet recognized standards, if not of beauty, but of attractiveness. Half a century ago, perhaps only celebrities could afford the luxury of changing their appearance by resorting to plastic surgery. Then a more affordable option appeared - contour plastic surgery, which was much cheaper, but also gave noticeable results. However, over time, more and more people began to turn to cosmetologists with the same request: to tighten, remove, correct. In a word – rejuvenate.

Mesotherapy is an important component of any cosmetic care, and in the case of an integrated approach to correcting age-related changes in the deformational type of aging, mesotherapy comes first.

There are common features of Slavic beauty and, so to speak, Slavic aging. The most visible deformation during aging in women of the Slavic type appears in the lower third of the face, while the aging of Asians follows a muscular type, which is basically due to the difference in the structure of skin tissue and the difference in the density of fatty inclusions. One of the characteristic features of Slavic women’s faces is often pronounced cheeks, which also affects the characteristics of aging. Under the influence of ptosis, the cheeks begin to droop over the years, forming “jowls” and a double chin, which require cosmetic correction.

With this type of aging, if contouring is immediately performed, especially in the middle and lower third, this can rarely fully solve the problem. The main group of fillers that aestheticians work with are hyaluronic, and they will “weight” the lower third of the face even more. Contour plastic surgery in this case will be an excellent final procedure for the entire complex of appearance correction, but definitely not the primary one. But to begin to solve the main problems of the deformational type of aging, which include hernial sacs under the eyes, excessive fullness of the oval of the face, overhanging nasolabial folds and the double chin area (lifting and thickening of the dermis after lipolytic mesotherapy), is through mesotherapy.

The first stage of mesotherapy correction: capillary mesotherapy

Whatever the causes and mechanisms of aging, impaired microcirculation is symptomatic of all types of aging. Improving the filling and speed of blood flow in the microvasculature allows you to relieve swelling and pastiness of the tissue. Effects on adipocyte beta receptors indirectly enhance lipolysis. Restoring venous outflow also improves the evacuation of metabolic products.

Capillar mesotherapy is an improvement of microcirculation using complex meso-cocktails, including revitalizing ingredients, together with vascular drugs - venotonics and capillary protectors.

The technique for administering the drug for capillary mesotherapy is classic: a needle 4 mm long - 30 G, the injection is made along the entire length of the needle along the paravertebral line between the spinous processes of the spine, two lines to the right and left of the midline. Legs and arms are processed using the same technique, with “straps” on the inside, also 2-3 lines.

Stage two: working with excess subcutaneous fat

The drugs of choice for mesotherapeutic lipolysis for deformational type of aging will be indirect lipolytics, which contain mannitol, taurine, buflomedil, theophylline, etc. It is this complex of active ingredients that effectively and safely eliminates swelling, blocks lipogenesis and stimulates lipolysis.

Correction of hernial sacs in the periorbital area

The periorbital area is a particularly delicate area, and you should always work with it with extreme care and caution. The presence of hernias also makes them difficult to correct.

In the periorbital region, only 4–6 injections are performed into each fat sac using a 4–6 mm needle – 32 G, along the entire length of the needle. Consumption – no more than 0.5 ml for both eyes.

Course: 1 time every 5–7 days, 4–6 procedures, 2–3 times a year.

The patient must be warned that after the procedure there will be slight post-traumatic swelling, which will go away within 1–3 days.

For complex age patients, additional correction of the nasolacrimal groove with a soft filler is possible.

Correction of nasolabial fold

Due to lipolytic mesotherapy in the area of nasolabial folds, the layer of hypodermis is minimized and overhang is smoothed out. Thus, in some cases, there is no need for contour correction of this zone at all.

The overhanging nasolabial fold should be treated with several injections of indirect lipolytic, 4-6 injections in each direction for the entire length of a 4-6 mm needle - 32 G, in a volume of no more than 1 ml. Course: 1 time every 5–7 days, 4–6 procedures, 2–3 times a year.

Correction of facial oval

The lower third of the face with this type of aging always responds gratefully when it is treated with cocktails with indirect lipolytics. The face noticeably loses weight and acquires a clearer oval shape, like in youth. Only after the volume of subcutaneous fat has been reduced and the pastiness caused by stagnant fluid has been eliminated, can the specialist begin contouring of this area.

Marionette wrinkles in such patients are often formed not by a crease, but by an excess of fat in the oval area. If the problem of excess fat is successfully corrected, the amount of filler injected will subsequently be significantly reduced.

When working on the oval of the face, the guideline is the line connecting the corner of the mouth with the tragus of the ear. We inject the area below this line with a 13 mm needle - 30 G, injecting the entire length or half the length of the needle, depending on the thickness of the adipose tissue. The distance between the injections and the lines is approximately 1 cm. Consumption per procedure is 2.5–5 ml. Course: 1 time every 5–7 days, 4–6 procedures, 2–3 times a year.

Double chin correction

The appearance of the so-called double chin is the most common complaint of patients of the Slavic type.

Mesotherapeutic lipolysis in the double chin area is the elimination of excess unaesthetically sagging hypodermis and a mandatory stage in preparing the oval of the face for lifting procedures. If the skin of the double chin is burdened with a thick layer of hypodermis, then lifting this area is very difficult and even impossible.

Technique: 13 mm needle - 30 G, injection - the entire length or half the length of the needle, depending on the thickness of the adipose tissue. The distance between the injections and the lines is approximately 0.5 cm. Consumption per procedure is 2–3 ml. Course: 1 time every 5–7 days, 4–6 procedures, 2–3 times a year.

If excess fatty tissue in the double chin area is especially pronounced, the esthetician can use a cocktail with direct lipolytic. Often this is a complex of phosphatidylcholine with sodium deoxycholate.

As for the dose of administration of direct lipolytic, there are certain restrictions, violation of which is strictly prohibited due to possible intoxication. The manufacturer always indicates the volume, which is not recommended to be exceeded.

Third stage: skin lifting

Full lifting is possible only by using specific cocktails with DMAE in combination with amino acids. DMAE, as a precursor of acetylcholine, creates a denser tissue texture, and the amino acid complex promotes neocollagenesis.

After successful lipolysis, the esthetician begins the stage of tightening and strengthening the facial skin to achieve a lifting effect.

Each subsequent procedure improves facial lifting; their number can reach ten.

Injection technique for drugs with DMAE: preferably a classic needle, 6 mm – 30–32 G. The distance between the injections and the lines is approximately 0.5–1 cm.

Injection technique in the middle third of the face

It is very important to place the injections along the Langer lines, the direction of the lines is from the center to the periphery, from bottom to top. This will ensure adequate skin retraction and a pronounced clinical result. If the sagging area of the second chin is significant, it is recommended to capture large border areas, up to the area of the upper lip and the middle of the cheek, while the line of injections should be raised to the tip of the ear. If the sagging is small, it is enough to treat the lower third of the cheek-zygomatic area, bringing the injection lines to the tragus of the ear.

Consumption for the procedure: for the oval – 3–5 ml, for the double chin – 2–3 ml.

Lifting procedures are prescribed once every 7–10 days and affect those areas where fat deposits were previously corrected.

Injection technique in the periorbital area

As for the periorbital zone, here for lifting or, rather, thickening the dermis, it is better to choose a cocktail with basic amino acids, vitamin C, and rutin. Amino acids stimulate the production of collagen and elastin fibers, vitamin C brightens well, and in combination with rutin has a beneficial effect on blood vessels, which further reduces the likelihood of edema and, as a result, re-formation of hernias.

Injection techniques in this area are micropapular and “nappage”, needle – 4–6 mm – 30–32 G.

The consumption of drugs is small - up to 1 ml for both eyes. Course – 4–6 procedures, 1 time every 5–7 days, 2–3 times a year.

In the periorbital area, hyaluronic acid should be abandoned in this case, since its moisture-retaining function will be inappropriate for such symptoms and will further aggravate the pastiness of the treated area.

When using the papule technique to administer drugs with DMAE, do not forget that the papules will be swollen and increased in size for some time. It is also necessary to prescribe this product with caution to patients with a history of allergic reactions, especially if they do not tolerate salmon fish. Before the procedure, it is necessary to conduct a test in the form of a subcutaneous injection and, after the result, evaluate the possibility of working with a group of these drugs in a particular case.

Conclusion

Thus, the Slavic type of appearance is successfully amenable to mesotherapeutic correction thanks to clear algorithms, protocols and correctly selected meso-cocktails. In just 6–8 weeks, it is quite possible to achieve the expected result without particularly expensive procedures and without surgical intervention, and to obtain a lasting effect, two or three courses per year are enough.
