Cosmetologist Inna Mirnova about fillers, Botox and facial aging
Let's find out with an expert how to prioritize when working with involutive facial changes.
How to choose the right drug for subcutaneous injections?
Any line of fillers for contouring includes preparations that differ in concentration, elasticity and dynamic density. Based on this, their characteristics in work are determined. There are soft, medium and dense preparations. Accordingly, the drug is selected according to the zones of administration.
Mild preparations are usually used for lips and superficial wrinkles. Medium-density preparations are used for deeper wrinkles, filling of nasolabial folds and other dermal creases, as well as for lip augmentation. Dense preparations are suitable for the cheek-zygomatic area, the formation of cheekbones, chin, nose.

There are three products in the Phillex line: Fine Lips - for young patients, as well as for lip contour to get a natural result; Deep Plus - more elastic and viscous, for lip augmentation, nasolabial folds (if necessary), forehead wrinkles after working with botulinum toxin and the middle cheek area; Sub-Q Plus - very well restores the volume of the cheekbones and temporal junction, suitable for rhinoplasty and chin. Phillex differs from other fillers in its new 4D matrix technology. This means that the hyaluronic acid molecule is better protected from destruction and remains in the tissues longer. Phillex Fine Lips lasts for 6 to 9 months, Deep Plus for 9-12 months, Sub-Q Plus for 12-18 months. By the way, it is now generally accepted that filler should not remain in tissue for too long, so it is very important that it biodegrades on average in a year and a half.
What options are there for injecting filler to remove a nasolabial fold?
The nasolabial fold forms with age due to the displacement of fatty bags. The center shifts to the middle and becomes heavier, so if a patient comes with a complaint about a nasolabial fold, they never, under any circumstances, start with it. First, they work on the formation of the cheek-zygomatic area. When it is raised, the nasolabial fold itself rises, and in most cases it remains natural.
There are patients who, from their youth, have deep nasolabial folds - not facial folds. In this situation, we use medium density fillers to fill in these wrinkles. As a rule, the middle zone is worked on first, and then, if it is really necessary, the nasolabial fold is worked on.
How to tighten the oval of the face when aging according to the “tired face” type?
In the case of a “tired” type of face, our entire ligamentous apparatus descends and lipodystrophy begins. That is, we lose subcutaneous fat. In this situation, medium-density or denser volumetric drugs are administered to restore the lost volume and support the ligamentous apparatus. The middle, zygomatic, buccal and temporal zones are restored. Remember that the cheekbone area never pricks without the temple.
How to smooth your forehead and eyebrow area?
If we are talking about forehead wrinkles and frown lines between the eyebrows, we first evaluate the patient’s facial expressions. If it is active, botulinum toxin is first used to turn off this muscle activity. And only then are fillers injected to more superficially fill the dermal creases if botulinum toxin has not smoothed them out.
It is not advisable to selectively work with filler during muscle activity, since fillers can move to the side, which does not look aesthetically beautiful. A superficial or medium activity filler is used. It is better not to use denser preparations, since there are a large number of vessels in this area, which can cause them to be squeezed.
