Correction of the lower third of the face: a plastic surgeon's approach

The lower third of the face is the area between the lines drawn at the base of the nose and in the lower part of the chin. Let's consider surgical methods of correction of involutional changes of the lower third of the face.
18-35 years old: we correct natural data
At this age, the problems of disproportion of the face usually come to the fore due to a disturbance in the development of the bones of the facial part of the skull. These can be conditions associated with anomalies in the development of the bones of the upper and lower jaw - retro- and prognathia, retro- and progeny. The presence of these conditions disrupts the ratio of thirds of the face, leading to the formation of a "bird's eye" face, visual dominance of the nose or, on the contrary, weighting of the lower part of the face, which causes aesthetic imbalance and, accordingly, discomfort in women.
36-42 years: we correct the initial signs of atrophy
In this age range, signs of atrophy and caudal displacement of facial fat bags, initial signs of ptosis of soft tissues, begin to appear:
- there is a caudal translocation of fat lumps of the cheeks and depletion of soft tissues of the middle face and periorbital zone;
- there is a lowering of the level of the oral cavity in relation to the upper row of teeth and the lower teeth become visible when smiling;
- the outer corners of the mouth may also be assumed.
As a rule, the contour of the lower jaw does not change directly, there is no sagging of the platysma on the front surface of the neck. Accordingly, the correction of changes in the area of the lower third of the face in this age category can take place "remotely", due to the elimination of ptosis of the middle area of the face, moving and fixing fat packs upwards.
